Private by default is the part I'd worry hardest about getting right — do you keep the model itself local, or is it more about never retaining what you send it? Those pull the architecture in pretty different directions.
Betting against training-hungry AI while everyone's business model depends on the opposite takes nerve. Private-by-default is the actual product bet here, not a checkbox feature.

The private-by-default approach is the right call. Most health apps treat your data like inventory to monetize, not something worth protecting. Solo ship means you own the whole stack, top to bottom. That gives you freedom most teams never get.
