New York City faces a supermarket shortage in several neighborhoods that is closely linked to health epidemics. At the request of the Mayor's office, the Department of City Planning studied supermarket need in the city. As a result, in April 2008, the Department found a widespread shortage of supermarkets, identifying several "food deserts." It identified food deserts as a key factor in why over 700,000 people in New York City had diabetes, over 1.1 million New Yorkers were obese, and another 2 million were overweight.[15]: 1

Physical restraint, particularly prolonged prone positioning with thoracic or cervical compression, may precipitate metabolic acidosis and restraint-associated cardiac arrest, even in the absence of significant hypoxemia. Experimental studies in healthy volunteers fail to reproduce real-world custodial conditions and should not be used to negate associated risks.

The King County medical examiner’s office has found that a man in Federal Way police custody at a local hospital died as a result of a heart attack and “prolonged restraint in prone position.” The medical examiner concluded 27-year-old Leon Alex Davis’ death was a homicide, according to a report released on Friday. “Alcohol withdrawal syndrome” contributed to the death. Federal Way police responded to St. Francis Hospital about 11:30 a.m. on March 14 after security personnel there reported a “disorderly subject,” identified as Davis. While en route, the officers received additional information that a security guard had been injured and that Davis was being held down, according to a news release from the Valley Independent Investigative Team. When they arrived, officers placed Davis in restraints and detained him. “After the restraints were applied, the subject experienced a medical emergency,” according to the news release. “Medical treatment was immediately administered; however, the subject did not survive.”

Letter to the Editor Male vagina is a more accurate term than prostatic utricle Furuya et al. stated “the prostatic utricle dilates in cases less exposed to male hormones during fetal development”. 1 The histology of so-called enlarged prostatic utricle is lined by squamous epithelium similar to that of the vagina, there- fore it should be termed a male vagina.2 The prostatic utricle is a cul-de-sac of the prostatic urethra, and extends backward and slightly upward for a very short distance within the middle lobe of the prostate. The prostatic utricle is 10–12 mm long, sometimes it is 20–25 mm, rarely it is 6–8 cm long. It is located in the seminal colliculus (i.e. veru montanum): this is a prominence of the dorsal surface of the prostatic urethra in which the two ejaculatory ducts open and among them exists the prostatic utricle; in addition, sometimes the ejaculatory ducts open into the prostatic utricle and not into the prostatic urethra. The urogenital sinus is divided into pelvic and phallic parts. The Emergency Department continues to be a space necessary for the concentration of technology, expertise, and administrative capacity to deal with critical situations. Yet, we can relieve some of the pressure experienced by staff working in the ED by engaging in the radical work of reconstruction to build communities with the capacity to meet people’s basic needs. What if instead of having to sleep on the streets or fighting for a bed in the ED, people were all guaranteed housing? What if young people were trained in basic first aid, the treatment of common medical ailments, harm reduction, and de-escalation in order to address these issues in their communities instead of relying on the ED? What if the locus of care was moved away from 134 the hospital to people’s homes and communities where many urgent situations could be dealt with in community? What if people were financially supported and educated in order to look out for the mental health of their loved ones and families? How do we work to offload the pressure we have placed on the ED to managed organized abandonment and participate in organized violence?
