Is it strictly signs or any extmark will do? Being able to set virtual text above the prompt with box drawing horizontal line would be epic.

Once this is builtin it opens the door for using normal-mode to editor prompts. Are you using PS1 / PROMPT_COMMAND to emit a sequence?
