You suggest that Finns’ attitudes towards trans identity in young people improved during the study period, undermining the minority stress theory. Do you have any documentation of that? This can be concluded for example from public discussions in the media and social media, rise of and great support to Pride movement across organizations, legal initiatives, visibility of sexual and gender minority issues in various contexts, students’ movements, adolescents’ increasing diversity of self-expression and their testimony in contexts where they meet (school, health and social care) professionals.
Is it possible that some of the specialist psych care that GR+ patients accessed was related to stress from a long delay in obtaining GR treatment? No, life stress alone is not a reason to be treated in specialist level psychiatric services.

“ As psychologist and advocate Diane Ehrensaft explained, the “gender-affirmative model” is defined as “allowing children to speak for themselves about their self-experienced gender identity and expressions and providing support for them to evolve into their authentic gender selves, no matter at what age.” This approach is promoted, despite its premise that gender “may be fluid” and “is not always binary,” and despite research showing that gender dysphoria in children usually resolves without medical treatment by adolescence — 85 percent of the time, according to the Endocrine Society. Why, then, are life-altering drugs given to children? “Gender-affirming care” is a big business. “Gender-affirming” centers servicing children have proliferated: There are now more than 400 pediatric gender clinics in North America, according to the Gender Mapping Project. The U.S. sex-reassignment-surgery market was valued at $1.9 billion in 2021 and is expected to reach $5 billion by 2030, expanding at a compound-annual-growth rate of 11.23 percent, according to Grand View Research. AbbVie reported $720 million in net revenue in 2019 from sales of Lupron, a powerful hormone suppressant approved initially for the palliative treatment of advanced prostate cancer, and which has been used to chemically castrate sex offenders. The Food and Drug Administration has also approved Lupron for the treatment of “precocious,” or abnormally early, puberty. However, “gender-affirming” physicians have prescribed it for preventing puberty from occurring at a normal age — a use the FDA has not approved. The director of GeMS received press attention for observing that puberty blockers are “being given out a lot like candy” in this off-label fashion.”
Dr. Jack Turban: To your point, these are big decisions to start these medical interventions. Doctors in this area follow two sets of guidelines. There's the Endocrine Society guidelines and also guidelines from this organization called WPATH, or the World Professional Association for Transgender Health. That's an international organization. Both guidelines emphasize that for minors, they need to undergo a comprehensive mental health evaluation before they consider these treatments. When we do that evaluation, we're looking for, are there any other mental health conditions this person has that need treatment or that could interfere with their ability to make a decision about this treatment? How long have they had gender dysphoria? To what degree is their gender dysphoria causing impairment? We have some patients who are so distressed by their gender dysphoria that they can't shower or they can't go to school.

Only a small fraction are the subset for whom the stakes are so high as you intimate: biological boys who have the option of undergoing pubertal suppression soon after the onset of puberty. The overwhelming share of youths who present to gender clinics these days are natal girls who arrive when they are already in the latter stage of puberty or who are finished with puberty. So, given the argument of having the opportunity to avoid the "wrong puberty" is put aside in most cases, this raises the question of why there is such urgency to start minors on such drugs, given young people's inability to consent to their own care.

West is skeptical, but she has always been disorganized, though high-achieving, and figures: Why not get a prescription for stimulants? Sadly, her health provider has other ideas. Dr. Buzzkill tells West that she will need to speak with her mother, because (as the DSM-5 states) ADHD is a disorder that is present from childhood. "I should have said no," West writes. "I was nearly forty years old at this point. My word on my own life should be sufficient, and under no circumstances should a medical professional need to call my mommy!"

Ben Ryan tweets that he believes 250,000 trans-girls are playing in girls' sports based on his previous tweet: "According to a 2023 HRC survey report, 19 percent of transgender and “gender expansive” youth played sports. While this rate is disproportionately low, the Williams Institute estimates based on a federal survey conducted that same year that some 724,000, or 3.3 percent, of 13 to 17 year olds identify as transgender. This suggests that perhaps 135,000 such middle and high-school students engaged in athletics at that time. An estimated 8.3 million high schoolers participate in sports according to the National Federation of State High School Associations. In multivariate analyses accounting for differences in follow-up times, the all-cause mortality of gender-referred individuals did not differ from that of controls when registered sex and year of birth were accounted for or when psychiatric treatment contacts were added to the model (table 2). Mortality was predicted by the male sex and an increasing amount of psychiatric treatment contacts.

Debunking podcaster Michael Hobbes either knows this or has chosen not to acknowledge it because it does not serve his interests (eg: it conflicts with his confirmation bias and his politics). As for Greg Gentry, he either has not read the Finnish study of suicide rates in youth attending gender clinics or did not understand it. Because if he had, he would know that the study was able to conduct a retrospective analysis of robust and voluminous national health data. There was therefore no need to conduct a study that would raise ethical concerns. Meanwhile, Hobbes breathlessly claims that there is a scandal of people falsely claiming that many youth receive gender-transition interventions after little or no assessment. He used to say there was zero evidence of this phenomenon. But after I reported that Boston Children's since 2018 has had a policy of providing kids seeking gender transition drugs with only a single two-hour assessment appointment a fact that the psychologist who founded its gender clinic told me was "shocking"), Hobbes switched to claiming that it's false that many kids get little or no assessment. He has never acknowledged that he was wrong. This is a man who used to have a podcast called You're Wrong About.
