Case Presentation • A 55-year-old male patient is followed for HFrEF and CKD stage IIIbA3. Other medical issues include obesity, hypertension, hyperlipidemia, and type 2 diabetes mellitus. • He is prescribed metformin, dapagliflozin 10 mg, losartan 100 mg, rosuvastatin 10 mg, and carvedilol 6.25 twice daily. Spironolactone 25 mg was recently introduced as part of GDMT. • Updated metabolic panel now shows hyperkalemia at 5.8 mEq/L with a slight creatinine "bump". He was advised to stop the spironolactone and restrict his potassium intake... • Familiar story?
