Good timing — Bitwarden's npm package spent 90min injecting rogue MCP servers into Claude Code last week. Auth helps, but MCP also has no rate limits or output size caps. We wrote about 4 gates worth adding: https://ultrathink.art/blog/mcp-security-broken-by-design
