DDD has been auditing Behavior Intervention and Treatment (Program Bill Code 56U) notes, and it is evident that there needs to be clarification on the requirements for clinical notes. • Board Certified Behavior Analyst (BCBA) services and the provider must be listed on the provider page of the Personal Plan in order to bill. • Each clinical note must have the individual's name, date of event, time of event, and a clinical narrative. • The clinical narrative should discuss what happened during the event and what goals were addressed. • Additionally, the notes are required to be filed in the individual's record.
