Sexually transmitted forms of Shigella usually come from direct or indirect contact between the mouth and anus. Between 2015 and 2020, sexually transmitted forms of Shigella rose 15% faster annually than variants transmitted through other means like travel or food. - More than 70% of the sexually transmitted strains of Shigella resisted at least 1 relevant antibiotic. - 1/3 of patients who contract sexually transmitted Shigella end up in the hospital, for an average of 4 - 5 days. - Up to 2/3 of GBMSM (men who have sex with men) with Shigella are also found to have other STIs, including HIV. Public health management of this issue is failing, as the advice for dealing with Shigella has been handwashing and food hygiene, which does not address how to prevent the sexual transmission of it. According to her research, older people are rarely asked about their sex lives, & that both healthcare professionals & younger people tend to assume that the older generation does not engage in sex. Another finding of this study is that sex education is needed both in youth & later in life, especially when it comes to doctors not doing their job properly, specifically when they do not tell their older patients about the side effects that certain medications can have on sexuality. Sexuality is varied in older people, with some using sex to combat loneliness, others who use toys & medication, & some not interested in having sex at all for various reasons. Shigella is an increasingly common bacterial infection that can spread through feces, usually via water contamination, unwashed hands, & certain types of sexual contact (esp. anus to mouth AKA rimming). A new strain with extensive drug resistance has been reported & the population being disproportionately affected is men who have sex with men. However, most cases don’t require treatment with antibiotics unless severe. After someone receives treatment for Shigella, they can remain contagious for a few weeks, despite no longer have symptoms. Recommendations to reduce the risk of transmission include: - Not having sex with someone who has diarrhea. - Not bottoming or getting rimmed by someone who has stomach issues or was recently treated. - If topping, washing off your penis thoroughly before putting it in another person's mouth or anus. Any fluid transfer involving saliva, semen, &/or blood can provide for transmission, so it is important to follow sterilization protocols for surfaces, implements, & toys to avoid transmission. PASS Sunscreen 101 Where should I apply sunscreen? Apply 2 fingers worth of sunscreen to your face & neck; reapply every 2 hours. ANY part of your body can get a sunburn, especially areas that don’t typically see the sun, particularly the popular TikTok trend of “perineum sunning.” Types of Sunscreen Mineral: The ingredients label will typically include “titanium dioxide” or “zinc oxide.” Although many dermatologists recommend this for people with sensitive skin, it can often leave a white cast. Mineral sunscreens work as a “shield,” creating a physical barrier between UV rays & your skin. Chemical: The ingredients label will not include titanium dioxide or zinc oxide. Specific “filters” can vary between American, European, & Asian sunscreens. Chemical sunscreens absorb UV rays. What Does SPF even mean? SPF stands for “Sun Protection Factor” which refers to how much solar energy (UV Rays) is required to produce a sunburn on protected skin. According to the Skin Cancer Foundation, “An SPF 30 allows about 3% of UVB rays to hit your skin. An SPF of 50 allows about 2% of those rays through.” Everybody should be using sunscreen! But there are some circumstances where it is especially important to remember to reapply sunscreen: Tattoos/ Tattoo Removal: Sun exposure can cause tattoos to fade. Laser Hair Removal: Like tattoo removal, any darkening of the skin can cause lasers to lose effectiveness. Herpes/ Cold Sores: Heat & sun exposure can cause breakouts. It’s important to remember to keep your medications close by in case of an outbreak.

Taking a dose of Doxy-PEP within 72 hours of sex is highly effective for men who have sex with men (MSM) & transgender women, who saw the steepest declines in infection. Doxy-PEP is far less effective against gonorrhea (only a 5% reduction) due to higher levels of existing antibiotic resistance. STI rates remain disproportionately high among Black residents, the unhoused population, & those using drugs. DoxyPEP is a 200mg dose of the antibiotic Doxycycline; if taken within 72 hours of a sexual encounter, it largely reduces the risk of syphilis & chlamydia infections, & also offers some protection against gonorrhea. One of the side effects of DoxyPEP includes an increase in your skin's photosensitivity, making sunburns more likely. Dr. Carlton Thomas advises people to ensure they wear sunblock, cover up, or avoid the sun after taking DoxyPEP. It is recommended that people who have recently taken Doxycycline, should avoid exposure to tanning beds & other similar light sources. Sexual health charity the Terrence Higgins Trust suggest taking doxycycline with fluid & some food, as well as remain upright for 30 minutes after taking a dose, in order to reduce potential side effects.
