A 62-year-old female with chest pain and shortness of breath was seen in your Emergency Department (ED), admitted to the cardiology ward and treated for acute coronary syndrome. During her recent hospital admission, she was also treated for heart failure and given doxycycline for a lower respiratory tract infection. Her past medical history includes diabetes, hypertension and a previous c-section. Two weeks after her hospital discharge, she returns to your ED with worsening confusion and is drowsy. She has a significantly reduced oral intake and is not passing urine. Her initial blood tests results show a raised serum creatinine and hyperkalaemia.

A 5-month-old child presents to the Paediatric Emergency Department with inconsolable crying. The crying episodes have become more frequent, which is why her parents decided to attended hospital, as this was very unusual for her. She has not had a fever. She has been passing urine without concern and is breastfeeding well. On examination, her abdomen is unremarkable and is soft non tender. Mum denies any diarrhoea or vomiting. However, 30 minutes after arrival, you are called to re-examine her. She has pallor, is inconsolable and you can palpate a mass in her right upper quadrant.

He was at a party the previous night and inhaled some substance from a cannister. His observations are as follows: RR 18 breaths/minute, heart rate 80 beats/minute, blood pressure 120/74mmHg, and oxygen saturations on air of 95%, with a normal ECG. On examination he has palpable crepitus on the anterior chest wall, and crunching breath sounds in the left hemithorax on auscultation. A 79-year-old female attends ED by ambulance after having 12 hours of central, pleuritic chest pain which radiated to the back across her shoulders. The patient presents in the morning having had pain all night. The pain had not come on suddenly. She felt a bit short of breath when the paramedics arrived but is not breathless on arrival in ED. She takes warfarin for a previous PE. Three weeks previously she had been admitted for 7 days and had had a permanent pacemaker inserted. She is in discomfort but not unwell. Observations are p78 regular, BP 150/90, T36.5, RR 20, sats 96% on air. On examination she has normal heart sounds, clear lung fields, she is slightly tender in the epigastrium and her calves are soft and non-tender. The pacemaker site is healing well and is non-tender. She is more comfortable sitting up in a chair.
