This earlier paragraph is fairly explicit (“the mutation of sexes, or transition into one another”) but uses “transexion” rather than “transfeminated”
To me this sounds like he’s describing intersex people where their status isn’t apparent at birth and he becomes more clear later in life, but I’m obviously not an expert
