While simply rotating an orthographic camera on its x-axis can maintain scale left-to-right, you still end up with foreshortening and a change in scale front-to-back. Instead of rotating the camera, keep that top-down, but add a custom normal to the displacement node and introduce some y-component.

This animation shows the difference between rotating the orthographic camera (first half) which produces foreshortening, and adjusting the Y-value of the input normal to the displacement node (second half).
