5/ 24 hours later I receive an "Appeal Rejected" with no explanation. The "resolution options" button, after a bit of clicking, takes you to a form that's basically exactly like the one I already filled in to appeal, albeit with I think 200 extra characters to use.

6/ I didn't even bother with this form, because HOW DO YOU PLEAD INNOCENCE TO AN UNSPECIFIED CRIME?!?! I just kind of went, "Oh well, I guess I don't have a YouTube account anymore" and moved on with my day. I had that luxury because I don't make my living off YouTube. Many others do!
